Why is soft tissue healing so important?
Fabbri: Repeated disturbance of soft tissue healing can cause apical migration of epithelial cells and subsequently unstable pockets of tissue around the abutment. And, of course, reopening the original wound also exposes the area to external bacteria. Unlike a healing abutment, the On1 concept’s key component – the On1 Base – is placed during surgery and not removed again. The undisturbed healing preserves the soft tissue adhesion.
What were your first impressions of the On1 concept?
The On1 Base raises the prosthetic interface of the restoration from bone level to a more coronal position. It helps us to optimize the prosthetic outcome while providing a more comfortable treatment plan for the patient. My first reaction was, “Why hasn’t anyone thought of this before?” There was great interest in this new concept from delegates at the Dubai symposium.
Why would you use the On1 concept instead of a traditional tissue-level implant?
If you compare the On1 concept to transmucosal implants, you will find multiple advantages. First of all, the On1 concept implements platform switching at the bone crest level to reduce vertical bone loss. Using Nobel Biocare’s conical connection implants with platform switching means that we can work flexibly in terms of the vertical position of the implant. Additionally, the two base-height options provide the versatility to select the ideal height in relation to soft tissue thickness.
Since we can choose between base heights, we can achieve an optimal situation for soft tissue adherence. In fact, it is possible to preserve a distance of at least 1.75 mm between bone and prosthetic interfaces, maintaining a suitable sulcus depth in relation to the clinical situations that we are treating. If we use a traditional transmucosal implant this may not always be possible.
You have been leading training in the On1 concept at Nobel Biocare Symposia. What has been the response of clinicians?
There has been a very good response from those who have tried the On1 concept. Many trainees at the symposia have been excited to see for themselves the biological, biomechanical and clinical advantages.
What are the benefits of attending the Nobel Biocare Symposia?
The Nobel Biocare Symposia are dental conventions that represent an excellent opportunity to stay up-to-date with implant dentistry trends and innovations both from the surgical and prosthetic points of view. Hands-on training is a crucial aspect of these events. Participants can work on animal or plastic models and try new surgical and prosthetic approaches, components and procedures and better understand what opportunities will work best for them and their patients.
Dr. Giacomo Fabbri graduated with honors in dentistry and dental prosthodontics at the University of Pavia. He is an active member of the Italian Academy of Prosthetic Dentistry (AIOP). He has published several international articles in peer-reviewed journals and lectured in Italy and abroad on topics related to fixed prosthodontics and implant prosthodontics with a specific interest in esthetics, new materials and minimally invasive approaches.
